Kids Club Signup

We thank you for entrusting your children into our care. We count this opportunity an honor and a privilege. We will strive to make their time with us enjoyable and safe. If your child's conduct is detrimental to the safety and well-being of others or staff, this privilege can be revoked. Our prayer is that we have your cooperation in this matter. Through careful instruction your children can experience an enjoyable and safe time.

Guidelines

Please initial each rule as you read and agree with them. This list is not inclusive.

No cursing or foul language.
No fighting; no pushing or shoving others.
Do not destroy property.
No disrespect to any Kids Club Ministry worker.

Not following the above guidelines can result in a suspension. If behavior does not change, your child will be prohibited from attending Kids Club.

By filling out this sheet, you consent to your child participating in Kids Club.
