Final Destination Waiver Form

Final Destination: The Church of the Living Dead, which is a ministry under the Bethel Baptist Church, is intended for your enjoyment. The consumption of drugs, alcohol, smoking, or the possession of explosives, fireworks, or weapons of any kind on the property is strictly prohibited.
Under-age drinkers will be arrested. Disorderly or aggressive behavior towards other patrons or volunteers will result in immedate ejection from the venue without a refund.
By participating in Final Destination and associated activities, you assume all risks and hazards associated with this event, and knowingly and freely assume the risk of any personal injury, property damage, or property loss that you may suffer, incur, or sustain which could in any way be construed to be connected with your participation in the activities at Final Destination. Final Destination is meant immerse and elighten while at the same time scare the audience, and assumes no responsibility to how you or others around you react to such scares. In addition, the walkthrough by design is poorly lit with many obstacles, and structures that are potential hazards.
You assume full responsibility in being fully prepaired with the proper attire for such hazards including proper footwear. Futhermore by signing this waiver you take full responsibility for your own actions and the actions of your children. Every minor must be accompanied by an adult authorized by that minor's parent or legal guardian to sign the waiver. Children under the age of 13 must have a parent or legal guarden accompany them during the entire event. The responsible party signing the waiver will take full resopnsibility for any injuries, property loss, or nightmares suffered while attending, or as a result of attending Final Destination. Upon entering the property you assume all risks and responsibilities, and waive Final Destination of any and all responsibilities, including, but not limited to permanent impairment and/or death.

By choosing to attend Final Destination, you agree that:

1). You have read and understand the above statement and agree to assume any and all risks associated with your decision to attend Final Destination.

2). You agree to release Final Destination and their successors, assigns, owners, officers, directors, volunteers, and property owners, of liability for any injuries or other harm that you and/or your minor children may suffer at Final Destination.

3). You understand by completing this waiver for vour self and minor children that it is a binding contract between you and Final Destination, operating under the Bethel Baptist Church.
